The Foundation aims to improve their neighbors’ health, inform their health decisions and empower them to lead healthier, more fulfilling lives.
The Horizon Foundation for New Jersey is committed to keeping New Jersey healthy by promoting health equity and cultural awareness in the diverse communities they serve. They will work alongside their partners to improve their neighbors’ well-being and address health disparities so everyone in New Jersey has the opportunity to live a healthy, more fulfilling life.
Three Pillars of Funding
- Caring – Advancing Education, Awareness & Disease Prevention: Support for evidence-based preventive programs that promote a healthy lifestyle for vulnerable and underserved individuals, families and communities.
- Obesity: Programs that promote healthy behaviors to reduce obesity and diabetes through increased education and preventive activities with a focus on:
- Healthy food accessibility
- Physical activity
- Nutrition education
- Public spaces
- Opioid: Programs that reduce opioid dependence and misuse through increased awareness, education, community engagement and peer support with a focus on:
- Mental health support
- Stigma
- Recovery
- Mental Health: Preventive programs that increase awareness, education and community engagement, and reduce stigma, especially among youth and adolescents, on mental health issues, with a focus on:
- Anxiety
- Suicide prevention
- Depression
- Other considerations will be made for programs that address trauma (Adverse Childhood Experiences) as a secondary focus.
- Grant range is from $10,000 to $50,000.
- Obesity: Programs that promote healthy behaviors to reduce obesity and diabetes through increased education and preventive activities with a focus on:
- Connecting – Reducing Barriers to Better Health: Support for evidence-based programs that address Social Determinants of Health. Programs coordinated by a health professional to help individuals overcome barriers to care and basic needs by addressing: access to health care, education, employment, food insecurity, homelessness, mental health services and transportation.
- Health professionals may include:
- Community Health Workers
- Nurses
- Case Managers
- Patient navigators
- Certified Peer Specialists
- Social Workers
- Grant range is from $10,000 to $50,000.
- Health professionals may include:
- Creating – Supporting Arts and Culture: General operating or programmatic support for the arts and cultural organizations to improve the quality of life for residents and communities throughout New Jersey. Support is by invitation only and is limited to organizations located and providing services in Burlington, Camden, Essex, Mercer and Monmouth counties.
- Grant range is from $5,000 to $15,000.
Eligibility Criteria
- The Horizon Foundation for New Jersey only considers requests submitted by organizations located in New Jersey that have tax-exempt classifications under Sections 501(c)(3) of the U.S. Internal Revenue Code. The Foundation will consider funding the state or regional office for organizations with multiple chapters or multiple affiliates operating in New Jersey. The Foundation will also consider funding local chapters or affiliates of organizations with state or regional offices by invitation only.
- Considerations for higher education applicants New Jersey universities and accredited colleges may submit one grant application per calendar year through their institution’s foundation.
- Organizations should meet the following criteria:
- Mission should promote health or the arts in New Jersey communities
- The program to be supported must align with the Foundation’s funding priorities
- Be able to identify specific and measurable short- and long-term objectives
- Must be located in and serve New Jersey communities
- Have demonstrated experience with the activities described in the funding proposal
- Should not duplicate the work of other organizations already supported by the Foundation within the same geographic area
- Have a current and satisfactory financial audit conducted by a Certified Public Accountant (required for all organizations, including those whose incomes are under $500,000)
- Agree to the Foundation’s financial and narrative reporting requirements for monitoring purposes
- Have a longstanding reputation for credibility and integrity
- The geographic location of the organization and communities served by the program will be taken into consideration.
- Specifically, grants to arts and cultural organizations will be by invitation only and limited to the following geographic areas in the state where Horizon BCBSNJ has office locations:
- Burlington County
- Camden County
- Essex County
- Mercer County
- Monmouth County
