The Maryland Department of Health (MDH or the Department), Office of Minority Health and Health Disparities (MHHD) is issuing this Request for Applications (RFA) to provide community-based interventions to address COVID-19 prevention, risk factor mitigation, and/or capacity building to prepare for future pandemic response.

Services will be provided in a specific jurisdiction(s) proposed by the Applicant with the goal of reducing COVID-19 disparities using one or more of the following strategies:
- General public information sessions about COVID symptoms, testing, quarantine and isolation protocols, and vaccination recommendations.
- Vaccine annual or semi-annual booster dose uptake promotion.
- Provision of linkage to health care services, linkage to social services, and direct provision of social services to persons directly affected by COVID-19 (cases and close contacts of cases).
- Provision of linkage to health care services, linkage to social services, and direct provision of social services to groups at high-risk for severe complications of COVID-19.
- Capacity and/or infrastructure investments that prepare community-based and faith-based organizations for response to future pandemics. A plan for the longterm maintenance of such capacity is required for this option.
Services will be provided in Maryland jurisdictions.
Funding Information
- The Department expects to make 15 to 25 awards of $50,000 to $200,000 each based on this RFA. Proposal budgets should not exceed $200,000.
- The anticipated duration of services to be provided under this Agreement is one year.
Target Populations
include racial and ethnic minorities and other disadvantaged populations.
Eligibility Criteria
The Applicant must provide proof with its Application that the following Mandatory Requirements have been met:
- The Applicant shall be a Social organization as defined per Section 7-402 of the State Finance and Procurement Article of the Annotated Code of Maryland or a local, state government agency, public college or state university.
- For social organization Applicants (not local, state government agency, public college, or state university), the Applicant must be a nonprofit organization, classified by the IRS as tax-exempt under section 501(c)(3) of the Internal Revenue Code. Applications must include attachments of the following documentation from the applicant:
- Documentation of tax-exempt status of the Applicant or the Applicant’s fiscal sponsor (i.e. IRS tax exempt status determination letter)
- Applicants must have a certificate of good standing issued by the Maryland State Government.
