Health Foundation of La Porte (HFL) is dedicated to the mission of empowering La Porte County residents to live healthy and well.
Health and wellness are at the core of HFL’s work. For HFL:
- Health is “a state of complete physical, mental and social wellbeing and not merely the absence of disease or infirmity” (World Health Organization).
- Wellness is the “active process through which people become aware of, and make choices toward, a more successful existence.” Wellness has six dimensions: emotional, occupational, physical, social, intellectual, and spiritual (National Wellness Institute).

- Grants up to $10,000
- Grants up to $10,000 have a simpler application form and reporting requirements to align with the amount requested (versus Grants of $10,001 to $25,000 or Grants over $25,000). Grant requests are accepted for proposals that address HFL’s strategic priorities of Healthy Children, Healthy Living, and Healthy Minds, as well as for Healthy La Porte grants. Healthy La Porte grants meet community health and wellness needs that fall outside of HFL’s strategic priorities and demonstrate a connection to HFL’s mission and vision.
- Grants of $10,001 to $25,000
- Grant requests are accepted for proposals that address HFL’s strategic priorities of Healthy Children, Healthy Living, and Healthy Minds, as well as for Healthy La Porte grants. Healthy La Porte grants meet community health and wellness needs that fall outside of HFL’s strategic priorities and demonstrate a connection to HFL’s mission and vision. - Grants over $25,000
- Applications to apply for a grant over $25,000 require a Letter of Inquiry (LOI) as a first step. Only applicants who receive a favorable response to their LOI will be invited to submit a full proposal. Grant requests over $25,000 must address one of HFL’s strategic priorities of Healthy Children, Healthy Living, and Healthy Minds.

Eligibility Criteria
To be considered for an award:
- The proposed program must directly serve or support residents of La Porte County.
- The proposed program must contribute to impact in one of HFL’s strategic priorities or respond to an identified community need to improve health and wellness.
Priority will be given to proposals that:
- Use evidence-based practices.
- Demonstrate a commitment to measurable results.
- Encourage collaboration between community organizations (if appropriate).
- Address needs for City of La Porte residents.
HFL Will Not Fund:
- Underwriting for an organization’s annual appeals/solicitations, special events, or general fundraising.
- Organizations that discriminate in any way based on participation or membership in a specific religious faith, race, gender, citizenship status, sexual orientation, age, or disability.
- Faith-based organizations will be considered so long as the grant funds are used for projects/programs that are offered to individuals regardless of their religious affiliation, race, gender, citizenship status, sexual orientation, age, or disability.
- Endowments.
- Requests from individuals.
- Associations of property owners, homeowners, condominiums, etc.
- Lobbying or the support of any political candidate, political party, or political activity.
