The Howard County Health Department, with support of state funding, seeks to fund community-based organizations (CBOs) to reach populations identified as having a known health disparity related to tobacco/vape use, diabetes, and chronic disease; and to advance health equity efforts. Project must address the chronic disease objective and one or both tobacco objectives:
- Chronic disease objective:
- Promote tobacco, diabetes and chronic disease intervention programs in communities with known health disparities;
- Tobacco objectives:
- Prevent initiation of use for all tobacco products among youth;
- Promote and provide referral to smoking cessation resources and services among adults and youth.
Funding Information
- Grant proposals may request up to $9,500.00
Eligibility Criteria
Applicants must meet the following eligibility criteria:
- A current non-profit or for-profit group in Howard County; and
- Registered and in “Good Standing” with the State.
