The purpose of the Healthy Life Grant program is to advance the health of their communities by funding new or expanded screening, educational, preventive, and wellness projects or by funding research that can be applied to such projects.
Funding Information
- The maximum individual grant amount is $2,500.
Guidelines
- Eligible programs will fall into the broad definitions of health, community betterment, education, disease screening, fitness/wellness assessment and/or improvement, or research.
- Services must be delivered to residents of the area served by Sauk Prairie Healthcare including Black Earth, Lodi, Mazomanie, Plain, Prairie du Sac, Sauk City, Spring Green, and others.
- Consideration will be given to programs that are finite in scope and duration and start-up projects that show uniqueness, creativity, and impact.
- They will consider matching funds for programs partially funded through other sources.
- Programs must commence during 2023.
- Programs or organizations which have been funded previously and have not filed the required reports will not be considered for funding.
- Consideration will be given based on the number of people served.
Eligibility Criteria
- Practitioners in all fields of health (excluding Sauk Prairie Healthcare)
- Educators, elementary and secondary schools, and school districts
- Community employers, civic organizations, and service clubs
- Government agencies
- Libraries, museums, and historical societies
- Ad hoc groups assembled for the purpose of undertaking a specific project. (Collaborative proposals are welcomed.)
