The purpose of this program is to provide substance misuse and HIV prevention services to racial/ethnic minority males at risk for HIV/AIDS. The program will place an emphasis on males who have sex with other males (MSM), including transgender individuals, as well as those who identify as LGBTQ+ who are not in stable housing and reside in communities with high incidence rates of HIV. Outreach will be directed to the population of focus to assist them in receiving HIV and hepatitis testing and support services.
The program will use a navigation approach (Community Health Workers, Neighborhood Navigators, and/or Peer Support Specialists) to expedite services for these populations. Organizations will have the opportunity to deliver comprehensive services (psychosocial support, medical care, housing, employment, family, education, and prescription drug assistance services) that will reduce morbidity and mortality for this high-risk population. The program will provide evidence-based substance misuse and HIV and hepatitis programming in hard to serve communities to reduce infections and increase protective factors using SAMHSA’s Strategic Prevention Framework.
Funding Information
- Proposed budgets cannot exceed $250,000 in total costs (direct and indirect) in any year of the proposed project.

- Length of Project Period – Up to 5 years.
Allowable Activities
Allowable activities can be supported with grant funds but are not required. Allowable activities may include:
- Developing and implementing tobacco cessation programs, activities, and/or strategies.
- Providing cultural competency and implicit bias reduction training to service providers to increase awareness and acknowledgment of differences in language, age, culture, socio-economic status, political and religious beliefs, sexual orientation and gender identity, and life experiences.
- Providing activities addressing behavioral health disparities and the social determinants of health.
- Providing mental health promotion and psychosocial support to include open access to flexible, early prevention intervention support for high-risk youth.
- Providing services to include screening for mental health, substance abuse disorders, HIV/viral hepatitis risk, and other psychosocial issues.
- Providing supportive counseling, motivational interviewing, referrals, and linkages to needed services and resources with a warm hand-off.
- Providing supportive services that address social determinants of health and childhood adverse experiences to prevent the onset of mental health (MH)/substance disorders (SUD) and reduce the risk for HIV/viral hepatitis.
- Providing early supportive services addressing the social-emotional needs of high-risk youth to increase resiliency and coping skills that prevent suicide.
Eligibility Criteria
Eligible applicants are domestic public and private non-profit entities.
