Eligibility Criteria
- Applicant must be a nonprofit, 501 (c) 3 tax-exempt organization; a school-based health program; or have a 501 (c) 3, tax-exempt organization as a fiscal sponsor.
- The request must align with one or more of strategic focus areas
- Building Health Equity– An equal and fair opportunity for every individual to attain the highest level of health and wellness regardless of socioeconomic barriers, and/or Increasing Wellness in Pajaro Valley The active pursuit of activities, choices, and lifestyles that emphasize preventing illness and improving quality of life, as opposed to emphasizing disease treatment.
- The request should serve individuals living in primary service area
In general:
- Community Health Trust’s Board prefers not to fund programs or projects administered by government agencies, with the exception of school-based health programs closely aligned with the CHT’s mission.
- While there is no formal policy about the number of consecutive years an organization is eligible to receive support from the Health Trust, grantees are typically invited to take a year’s hiatus from applying after four consecutive grants.
