Funded grants pilot and evaluate new or expanded community-level, dementia-capable HCBS programs. Recipients of these grants must use at least 50% of total grant funds for direct services. This NOFO includes two application options:
Option A: Grants to states
Applicants for Option A are the governmental entities within states and territories designated as the state agency for dementia capability.
Option A has two required objectives:
- Objective 1: Create, expand, and sustain a dementia-capable state HCBS system that includes Single Entry Point/No Wrong Door (SEP/NWD) access for people living with dementia and their family caregivers
- Objective 2: Ensure access to a comprehensive, sustainable set of quality state HCBS systems that are dementia-capable and provide innovative services to people living with dementia and their family caregivers
Option B: Grants to communities
Applicants for Option B are public and/or private community-based organizations that operate within an existing dementia capable HCBS system. Eligible applicants must be able to articulate opportunities for new partnerships and opportunities to expand and implement proposed programs (services and supports) in their existing system.
Option B requires applicants to address all three of the following gap areas:
- Provision of effective supportive services to persons living alone with Alzheimer’s disease and related dementias (ADRD)
- Improvement of the quality and effectiveness of programs and services dedicated to individuals aging with intellectual and developmental disabilities (IDD) with ADRD or those at high risk of developing ADRD
- Delivery of behavioral symptom management training and expert consultations for family caregivers.

- Expected funding: $47,537,320
- Funding range: $850,000 to $1,000,000
- Option A: Minimum $850,000, maximum $900,000
- Option B: Minimum $900,000, maximum $1,000,000
Project Period
Both Option A and Option B are fully funded at the time of the award, for a 36-month (three 12-month budget periods) period of performance (2024–2027).
Eligibility Criteria
Only the following types of organizations may apply:
For Option A
State governmental agencies designated to serve as the State agency on dementia capability that do not have active ADPI grants.
For Option B
Domestic private or public nonprofit, community-based organizations (CBOs) able to demonstrate that they meet all the following criteria:
- Currently operating within an existing dementia capable HCBS system dedicated to the population that they serve
- Identifying and fostering opportunities for new partnerships, educational opportunities or expanded or additional services and supports in the targeted gap areas that would enhance and strengthen the HCBS existing system, as well as the system’s infrastructure and ability to implement such activities
- Already engaged in working relationships with their state agencies charged with creating and sustaining dementia capable HCBS systems
For both Option A and Option B, only the following categories of entities are eligible to apply:
- State government
- County government
- City or township government
- Special district government
- Regional organization
- U.S. territory or possession
- Public/state-controlled institution of higher education
- Indian/Native American tribal government (federally recognized)
- Indian/Native American tribal government (other than federally recognized)
- Indian/Native American tribally designated organization
- Nonprofit with 501C3 IRS status (other than institution of higher education)
- Nonprofit without 501C3 IRS status (other than institution of higher education)
- Public/private institution of higher education
