In accordance with The Weld Trust’s vision to help Weld County become the healthiest County in the State of Colorado, the second quarter 2022 initiative is focused on: Healthy Habits
- The Weld Trust will strive to remove the most significant barriers to access so that Weld County citizens of all ages can build healthy habits.
- The Weld Trust will fund programs as well as informational campaigns around Weld County to encourage the early development of healthy habits including exercise, nutrition, and preventative health care.
In addition, The Weld Trust gives priority to projects that:
- Address underlying social and economic challenges contributing to poor health outcomes in the communities served, such as poverty.
- Extend past individual behavior or program-related services by modifying policies, systems, and environments to help make healthy options practical and readily available.
- Demonstrate collaboration among sectors, including public health, government, healthcare, etc. and among multiple nonprofit agencies/institutions/departments/governmental entities.
- Prioritize engagement of residents from the communities served.
- The Weld Trust values evidence-based best practices, but also encourages innovative solutions.
- Leverage additional funding as well as community assets.
- Have a demonstrable sustainability plan that will enable the work to continue beyond the funding period.
- Provide an established plan for evaluation including measurable outcomes. Explain the way in which you will know that you have been successful in achieving the impact you intended. Identify the specific manner in which the outcomes you propose impact the Healthy Habits funding strategy of TWT listed above.
Eligibility Criteria
Eligible applicants must be tax-exempt organizations under Internal Revenue Service 501(c)(3) or operate under the fiscal sponsorship of a tax-exempt nonprofit, including schools and public organizations.
PLEASE NOTE: Only nonprofit organizations, schools and government agencies located in Weld County or whose program is exclusively for Weld County residents in the areas of health or education are eligible to apply
