UWBC is releasing this RFA seeking established evidenced based, promising, or replication of successful local models, which are results oriented and have a clear set of measurable goals, tasks, and objectives in the following service areas:
- Health and Wellness In or Out of Home Care (including medical and preventive services)
- Health and Wellness Care Coordination
Organizations may apply for each area as a standalone application or a combination. For example, a combined Health and Wellness In or Out of Home Care with Care Coordination or Care Coordination.
UWBC is interested in funding evidenced based, promising, or replication of locally successful programs models that are targeted community based programs that provide one on one or face to face health and wellness care coordination, which meet one of the following outcomes:
- Increase access to needed health and wellness resources
- Maintain or increase the patient/clients’ level of functioning
- Increase family functioning
- Increase well-being
UWBC is seeking creative, evidence-based, promising practices, or replication of successful local programming to improve the health access and health status of vulnerable populations residing in Broward County. Services must be results-oriented with clear measurable goals, tasks, and objectives. Programs must focus on one of the following program areas: health and wellness in and out of home care or health and wellness care coordination.
Applications must demonstrate a knowledge of the current health climate of Broward County and resources available in the community to address community health conditions with the intention to align or collaborate/partner with those identified and existing sources
Grant Period
Funds will be awarded for FY2023/2024, beginning July 1, 2023 and ending June 30, 2024, with the possibility of renewal for two additional years.
Eligibility Criteria
For fiscal year 2023-2024, UWBC will be accepting applications from non-profit agencies that have been determined by the Internal Revenue Service to be categorized as agencies created for a charitable cause—most commonly referred to as 501(c)(3) organizations.
