This grant allows health and wellness programs in Barnes County the opportunity to apply for grant funds to provide low-income individuals the opportunity to participate in health and wellness programs.
The overall goal and purpose of the Community Wellness For All program is to increase physical activities and integrate multiple aspects of wellness for low-income individuals of Barnes County.
Eligibility Criteria
- Charitable organizations or programs may apply and should submit either:
- 501c3 determination letter from the IRS or,
- Fiscal Sponsorship Commitment Form if you do not have tax-exempt status from the IRS or,
- Provide organizational documents showing how your organization meets the charitable requirements which can include Mission and Vision, Bylaws, and/or Articles of Incorporation.
- Only organizations and programs that operate in and serve Barnes County residents are eligible to apply.
- School-supported activities are excluded from receiving grant funds.
Requirements
- Organizations are required to establish their program and protocol for individuals to apply for scholarships as laid out in the grant application.
- The foundation requires that the individuals receiving scholarships pay a portion of the costs which is developed through your application and program.
- Organizations shall provide quarterly reports to SVCF by using a template that will be provided. This is to ensure the funds are being used as stated in the application. The SVCF holds the authority to cease grant funding if the charitable intent is in question. Attendance, cost share of scholarship recipient and verification of need will be strongly monitored through these reports.
- Funds will be dispersed quarterly after review of the quarterly reports submitted.
- Grant funds are intended solely for scholarships to offset the cost for an individual to participate. Grant funds shall not be used for general operating or to pay for equipment or supplies necessary to participate.
- Although matching funds are not required to receive grant funds, they encourage applicants to demonstrate broad community support for the program. This may include examples of an active board or other funding sources.
