The CareSource Foundation, through the CareSource Foundation Grant Challenge, would like to recognize those community-based organizations and nonprofits that are diligently working to improve the health and well-being of mothers, infants, children and their families across the state.

Nominees should be organizations or programs that:
- Improve infant and maternal health by increasing access to prenatal care earlier in pregnancy, postpartum care and maternal/child/adolescent mental health supports.
- Reduce health risks that lead to preventable pregnancy-related complications such as maternal diabetes, hypertension, depression and substance abuse.
- Use a data-driven and community-based approach to advance interventions that reduce health disparities among communities of color.
- Promote strong families and communities by creating safe, stable and nurturing environments that reduce violence and the risk of injury.
- Support early childhood learning and development through regular access to ongoing health care, proper nutrition, regular sleep and a healthy amount of physical activity.
- Address Social Determinants of Health to improve the quality of health care for children, adolescents, pregnant women and families.
Examples of organizations or programs that nonprofits are managing that would qualify for this grant challenge:
- Initiatives and strategies that improve outcomes for children and families.
- Support services that are family-focused, person-centered, culturally-responsive and trauma-informed.
- Initiatives that expand the use of emerging or best practices that are evidence-based.
- Organizations that improve patient care, experience and engagement.
- Initiatives that integrate and enhance the safety-net system through partnerships or innovative models.
- Programs that facilitate better care coordination and navigation for children and families.

- First place: $50,000 award (1 organization)
- Second place: $30,000 award (1 organization)
- Third place: $20,000 award (3 organizations)
- Semi-finalist nominees receive $5,000 (5 organizations)
Eligibility Criteria
- Organizations must be a 501(c)(3) tax-exempt nonprofit entity, according to the Internal Revenue Code.
- The organization’s program must be delivered in at least one of the following service delivery areas and counties listed below:
- Harris Service Delivery Area – Austin, Brazoria, Fort Bend, Galveston, Harris, Matagorda, Montgomery, Waller, Wharton
- Jefferson Service Delivery Area – Chambers, Hardin, Jasper, Jefferson, Liberty, Newton, Orange, Polk, San Jacinto, Tyler, Walker
- Harris Service Delivery Area – Austin, Brazoria, Fort Bend, Galveston, Harris, Matagorda, Montgomery, Waller, Wharton
The following programs or services are not eligible for nomination:
- Reimbursable services
- Capital campaigns or renovations
- Feasibility studies
- Marketing or awareness campaigns
- Political or election-related activities
- Research studies or projects
- Promotional, social or community events
- Individuals
- For-profit enterprises or activities
- Activities that exclusively benefit the members of sectarian or religious organizations
- Organizations established for fewer than 2 years
- Organizations managed by a CareSource employee or board member
- Organizations managed by a family member of a CareSource employee or board member
Nomination and Selection Criteria
- Nominees do not need to be based in the state competition they are entering as long as the program has a physical presence in the state (employees or office) and the nominated program impacts adults and/or children who live in that state or county/region noted for the program.
- While a nonprofit’s nominated program may be comprised of partners that are not nonprofit organizations, the nominated organization must be a 501(c)(3) tax-exempt nonprofit entity, according to the Internal Revenue Code.
- Two nonprofit organizations supporting the same program are not eligible to both be nominated – only one of the nonprofit organizations should apply.
- All questions on the nomination form must be filled out unless marked “not applicable.” Incomplete forms will not be reviewed. Nominations may be rejected if they do not sufficiently and specifically describe the activities and impact, or if they do not align with the goals of the competition, according to the CareSource Foundation’s discretion.
- All nominations must be completed electronically using the link below. Nominations submitted via email will not be accepted.
- All nominations will be reviewed by a CareSource Review Committee, comprised of a cross-functional multi-state panel of experts and CareSource Foundation staff. The CareSource Review Committee will review all relevant information submitted by the nominees to make a determination as to which of the nominees are the most qualified recipients, meet the criteria, and best support the goals of this Grant Challenge and the CareSource Foundation mission.
- The top qualified nominees, as determined by the CareSource Review Committee, will be moved on to the public voting process that will determine award winners. CareSource will notify the top nominees of the decision prior to them being announced as eligible for the public voting process. During the public voting timeframe, residents can vote for the organization or program they believe makes the biggest impact on improving health. The public can vote once a day for the duration of the voting period.
