This Request for Applications (RFA) is an effort to support the work of community-based organizations serving economically disadvantaged minority groups and minority group members, especially those organizations that are located in areas where health disparities are high. The 2023-2024 Public Health Information Campaign (PHIC) Grant will fund only one project from one community-based organization that raises public awareness of health disparities and health equity issues impacting economically disadvantaged minority communities and engages impacted communities on developing solutions and strategies regarding policy, system and environmental changes that would improve health and wellbeing.
The goals of the work to be funded through this grant is to help improve public awareness of factors impacting health status and resources available to eliminate health disparities among economically disadvantaged minority groups in Wisconsin through the promotion of individual, community, and population health. The five priority areas for activities includes:
- Addressing social and community conditions, such as: economic well-being, supportive systems of dependent care, healthy housing,
- Physical, mental, and systemic safety,
- Person and community-centered health care,
- Social connectedness and belonging, and
- Mental and emotional health and well-being.
Funding Information
The Public Health Information Campaign Grant will be disbursed from the Minority Health Public Health Information Campaign appropriation. This year’s grant award is up to $50,000. The contract shall be effective on July 1, 2023 and shall run through June 30, 2024.
Eligibility Criteria
- Only private nonprofit corporations can apply.
Only proposals that include all the following elements will be considered for funding. Your proposal must:
- Clearly indicate which community (or communities) of color will directly benefit from your project. The four key community groups are: Black/African American, American Indian, Hispanic/Latino, and Asian American. (Wisconsin Legislature: 250.20)
- Specify at least one priority area from Wisconsin’s State Health Improvement Plan, that your project will incorporate or impact. The current five priorities are Social and community conditions (economic well-being, supportive systems of dependent care, and healthy housing); physical, mental, and systemic safety; person and community centered health care; social connectedness and belonging; and mental and emotional health and well-being.
- Give a clear projection of the number of people you plan to reach during the event(s)/project(s).
- Include an evaluation plan for the project.
