Community Health Connect is a cross-sector collaborative in Williamson County set on improving health outcomes and expanding resources for individuals experiencing health risks by implementing a Pathways HUB (HUB) model to build a network of Community Health Workers (CHW) serving individuals in Health Equity Zones across the county. United Way for Greater Austin serves a backbone agency for Community Health Connect. To support this work, United Way will fund nonprofit organizations and government entities to serve as Care Coordination Agencies providing CHW services for pregnant and postpartum mothers living in Taylor and Eastern Williamson County in alignment with the Pathways Community HUB Institute® (PCHI) model.
Funding Information
United Way is seeking applications for grants between $160,000 and $340,000 to provide CHW Care Coordination services in Year 1 starting November 1st, 2023. It anticipates a total of three years of funding.
Eligibility Criteria
- Operate as a registered nonprofit organization and have an official letter of determination of 501c3 status from the Internal Revenue Service or operate as a local government entity providing services to the public
- Not be delinquent on any state or federal debt.
- Accept the terms of the United Way Grant Agreement
- Share Community Health Connect’s and United Way’s view of collaborative and inclusive service to the entire community, without excluding anyone on the basis of race, color, religion, gender, ethnicity, national origin, age, disability, sexual orientation, or any other factor not relevant to a person’s eligibility for service to ability to contribute
