Through this funding opportunity, IHC is making approximately $15 million available. Applicants are encouraged to consider innovative projects that will modify, enhance and expand healthcare services to underserved communities and populations. Additionally, organizations with a demonstrated financial match will receive extra consideration.
This RFA seeks applications which focuses on providing services to those Polk County qualified residents eligible to receive the services based on their domicile, limited income, and inability to obtain sufficient third-party insurance or to qualify for other state or federal programs that cover their medical needs.

Eligibility Criteria
The grant is open to healthcare-related, non-profit organizations.
Required Qualifications & Certifications
Applications must demonstrate:
- The organization provides health, mental health, behavioral health, educational or other healthcare-related services.
- The organization has knowledge of Polk County communities/areas disproportionately affected by health issues including any emerging trends and population demographics.
- The organization has all insurance coverages, permits, licenses and professional credentials necessary to perform the services specified in this RFA.
