Proposal Focus
Outreach – Provide grant funds to support wages, benefits and equipment and training for new Community Health Worker (CHW) positions CHW to serve older adults age 60 years and older and their family caregivers with special focus on reaching underserved and diverse populations in in Stearns, Benton, Sherburne, Wright, Todd, Wadena, Morrison, Cass, Crow Wing, Mille Lacs, Isanti, Chisago, Pine and Kanabec counties. The funding is limited to supporting new
positions; indirect costs are allowed.
