Applications for program grants to provide structured quality of life programs (cultural, music, & the arts) as well as services for elderly populations, including increased awareness and training regarding proper safety measures, tips, and tools. Programs should encourage participation by health care professionals as well as family members with special emphasis on population gaps in underserved communities.
Proposals must address the scope of the program; number of patients it will help, how access to the program will be achieved; how patient progress will be monitored and tracked; and how outcomes will be measured.
Funding Information
Proposals up to $25,000 will be considered.
Eligibility Criteria
Applicants must be a 501©3 Rhode Island nonprofit in good financial standing; grants will be awarded to organizations, not individuals. Preference given to new proposals addressing critical gaps in unmet need as described above. Incomplete proposals will not be considered.
Requirements
In addition to the proposal description, please include a 12-month proposal budget, a current fiscal year organizational budget comparing actual revenue to expenses budgeted, a cover letter including a summary of your proposal, and the last two audits.
The 12-month proposal budget should include:
- all income for the project that the organization currently has on hand, and funding that is anticipated;
- break-down of all personnel by position and percentage of time that the employee will devote to this project (a class of employees may be grouped together).
