The Owensboro Health Community Health Investments Grant Program provides financial assistance to nonprofit organizations, governmental or public entities who are working to address identified community health priorities based on their local community health needs assessments and/or community health improvement plans.
Owensboro Health seeks and supports partnerships to impact the significant health issues facing their communities. As tobacco use is a factor related to multiple health issues in their region and state, it is their decision to make opportunities for investments to those organizations, like Owensboro Health, to those which have committed to establishing and implementing 100 percent tobacco-free policies.
Funding Information
A mini grant request may be made for an amount not to exceed $2,500.
Guidelines
- 501(c) 3 organizations, government and public entities are eligible to apply.
- Grants to Section 501(c) (4) organizations may be considered if the proceeds are used exclusively for section 501(c) (3) permissible purposes, are not used for lobbying, and are not used for political campaign activities. A 501(c) (4) recipient must report back to Owensboro Health on its use of the grant proceeds. Further, grants to 501(c) (4) organizations will not be permitted to cover the recipient’s fundraising costs or to provide general support. A 501(c) 4 recipient, like all grant recipients, must provide a mid-year and end of year report to Owensboro Health on its use of grant proceeds.
- Organizations cannot apply to both Owensboro Health Regional Hospital and Owensboro Health Muhlenberg Community Hospital mini-grant programs. Organizations in Muhlenberg County must apply for Owensboro Health Muhlenberg Community Hospital mini-grant
- Programs for which funds are requested must focus on providing activities or services that address an identified health need or that address root causes of health problems, social determinants of health. The connection between health needs and the activity or service must be clearly stated on the application. Only completed applications will be considered.
- All requests must include a specific project/fundraising budget.
- Geographic area: requesting organizations must seek to impact citizens within the following eighteen (18) counties in the Owensboro Health service area: Breckinridge, Butler, Daviess, Edmonson, Grayson, Hancock, Hardin, Hart, Henderson, Hopkins, Logan, McLean, Muhlenberg, Ohio, Union, and Webster Counties in Kentucky and Spencer and Perry counties in Indiana.
- If an organization wishes to recognize Owensboro Health as part of a project or program, an explanation in writing of your plan to do so must accompany the application prior to the project or program.
- Any use of the Owensboro Health logo must be consistent with Owensboro Health graphic standards. Any use of Owensboro Health’s logo must be approved by Owensboro Health Marketing and Public Relations
- In some instances, Owensboro Health may request advance review written copy for invitations, advertisements, press releases, posters or other promotional information related to the event.
- Applications will be approved or declined based on compliance with guidelines, program impact and available funding.
- Applications must demonstrate a commitment to provide services regardless of race, religion, sex, age, disability or national origin.
- Partisan political groups or activities, endowments, memorials, individuals, field trips or tours are not eligible for funding.
- Applications involving a collaborative effort with other partners will be given a priority.
- To avoid duplication, requests from national organizations should be coordinated between the regional and county affiliated offices within the Owensboro Health regional service area.
- Owensboro Health will only consider applications by schools and school-affiliated groups and activities that have been authorized by the school’s central office. Please include proof of authorization from your school’s central office with your application.
