The Village model is a membership-based organization with the goal of allowing community members to reside in their own homes while being able to access services that address their changing lifestyles as they age. The Community Bridge model is built on a dedicated community services site, blending in off-site service coordination, development of social networks, and recruitment of volunteers for service delivery; and serves a distinct neighborhood area.
Target Population
This program is designed to serve all ethnicities and populations, with focused expertise to promote the unique cultural needs which have been identified as demonstrating the greatest economic and social need:
- Persons with low income
- Persons who are socially isolated
- Persons with limited English-speaking proficiency
- Persons from communities of color
- Persons who identify as LGBTQ+
- Persons at risk of institutionalization
Funding Information
- The total estimated annual funding for this RFP is approximately $801,338 per year, which may increase or decrease depending on funding availability.
- Project Period: A grant awarded pursuant to this Solicitation shall have a tentative term of four (4) years from July 1, 2023 to June 30, 2027.
Client Eligibility
To be eligible for services, clients must be:
- An older adult aged 60 years or older; or
- An adult with a disability, aged 18-59; and
- A resident of San Francisco, and
- Willing to pay annual membership fee to participate in a Village program.
