A Community Connector program serves a distinct neighborhood otherwise lacking in dedicated community center space. This program provides healthy aging activities, socialization opportunities, one-to-one assistance, and other services to support the health and independence of older adults and adults with disabilities.
Funding availability for this RFP is intended for programs that follow the Community Connector service model and is specific to neighborhoods that lack a dedicated community center space. This model aims to develop a sense of community and promote networks in which neighbors support each other, services are led by community members, and service location is community based and utilizes public and private space. Through this service model, activities are managed by a neighborhood resident serving as the “Community Connector”. This Community Connector also facilitates neighborhood outreach to develop a volunteer network.
Target Population
This grant is designed to serve all ethnicities and populations, with focused expertise to promote the unique cultural needs which have been identified as demonstrating the greatest economic and social need:
- Persons with low income
- Persons who are socially isolated
- Persons with limited English-speaking proficiency
- Persons from communities of color
- Persons who identify as LGBTQ+
- Persons at risk of institutionalization
Funding Information
- The total estimated annual funding for this RFP is approximately $711,121 per year, which may increase or decrease depending on funding availability.
- Project Period: A grant awarded pursuant to this Solicitation shall have a tentative term of four (4) years from July 1, 2023 to June 30, 2027.
Eligibility for Program Enrollment
To be eligible for services, clients must be:
- An older adult aged 60 years or older or
- An adult with a disability, aged 18-59 and
- A resident of San Francisco and
- In need of Community Connector services
