The Ryan White Part A Program at Public Health – Seattle & King County (PHSKC) funds the Lifelong Dental Program (LDP). LDP is a comprehensive oral health access program whose aim is to support low-income HIV positive individuals living in King, Snohomish, and Island counties with access to oral health care services. The program covers the costs of dental procedures that are deemed medically necessary and time sensitive by participating dental providers and clinics. Program funds can be used to treat Phase I – Urgent/Diagnostic, Phase II – Disease Control (only when imminent), Phase III – Restorative, and Phase IV – Maintenance dental procedures as identified per LDP Dental Fee Schedule.
Eligibility Criteria
Note: Applicants that do not demonstrate meeting the following requirements will be deemed ineligible.
- Must be licensed to provide dental services in Washington State and licensure must be current.
- Must have professional liability insurance that conforms to PHSKC requirements.
- Must be willing to sign LDP’s Memorandum of Understanding (MOU – Appendix A) and provide all required materials outlined in the MOU
- Must be willing to adhere to the LFP fee schedule. Revisions to the fee schedule may occur during the contract year.
