Baltimore’s historically redlined communities still experience great disparities in critical, health-related resources like food, emergency assistance, transportation, and mental health services. The inaccessibility of these resources drives health disparities, including COVID-19 vaccination rates. The Baltimore City Health Department (BCHD) and Civic Works are awarding mini-grants of up to $25,000 to community-based organizations in Baltimore City to:
- Increase COVID-19 vaccination access and education in underserved communities.
- Expand and improve access to health-related resources.
- Improve the health and well-being of Baltimore City residents.
Program Breakdown
Social Determinants of Health (SDoH): Expand Baltimore residents’ access to one of these resources (food, transportation, housing, cash assistance, employment, mental health, substance use, education, utilities, healthcare) by:
- Helping people locate, apply, and obtain existing community resources.
- Filling a gap. Provide additional resources, especially those that are in high demand.
- Make it easier to apply, access, or obtain existing resources by expanding hours, making applications simpler, or adding locations.
Successful SDoH applicants will:
- Show their experience with one of the following: 1) providing navigation or 2) providing resources.
- Tell us about a problem or resource gap and how they plan to solve it.
- Tell us how many people they will connect to resources.
Funding Information
Foundation will award 5 $10,000 grants and 3 $25,000 grants to projects operating over a 3-month activity timeline.
